I have a combined payment for some items for which the combined payment was made through PayPal.
The PayPal IPN for one of those items has a different email address that the payment was sent to than the email address that I used in the AddItem call.
What is going on?

When two or more items are processed by PayPal in a combined payment, the PayPal email address that the payment is assigned to is the first item in the list, with the first item being defined as the first item that ended.

Let's say you listed 2 items with different PayPalEmailAddress values in each AddItem call:

7500001001 - ended at July-11-05 15:46:46 PDT - the paypal email address in this listing is joe@mycompany.com
7500100102 - ended at July-11-05 15:56:00 PDT - the paypal email address in this listing is jack@mycompany.com

Since the first item in the combined payment that ended has a paypal email address of joe@mycompany.com, the transactions under that combined payment receive that email address since PayPal will process the combined payment with that email address as the payment destination.

The IPN for item 7500100102 will show that the payment went to joe@mycompany.com
